Welcome to Minnesota Oncology, we look forward to providing you with high quality care and service. 

During this appointment you will complete all necessary paperwork, review your health history with a nurse and meet with your physician. 

Please make sure to bring the following items with you: 

• Insurance cards or payment information
• Completed New Patient Health History form
• If you are not able to complete the health history form ahead of time, please bring a list of all medications (prescription and over the counter, vitamins & supplements) with you. The New Patient Health History form will still need to be completed (in the office) because this information is very important for your physician to have. 
• Prescription Card 
• Completed Race/Ethnicity/Language form

Please plan on spending at least two hours in our office to allow for your appointment time and any arrangements or future appointments that your doctor may order. We also have Patient Financial Counselors available to answer any questions you may have about insurance and payment issues. 

Please be aware that if you are late for your appointment, we may need to reschedule you for another time. We do our best to see all patients in a timely manner. If we experience unexpected delays, we will keep you updated in the event you need to make other arrangements as well.
